Abstract : 「緒言」不正咬合に起因する咀嚼器官の機能不全がある場合, その咬合再構成を行うことによって正常な機能を回復することは, 歯科矯正学にとって重要な役割である. この際, 咬合面形態, 特に歯軸の傾きによって変化する咬頭傾斜角は, 下顎運動と密接に関係しているため, 顎口腔系, すなわち, 歯, 顎関節, 歯周組織, 神経筋機構ならびに脈管系の相互作用をも考慮して決定されなければならない. 歯科領域において, これまで咬合面形態や咬合接触について多くの研究がなされてきたが, 近年, 顎運動についての研究が多方面からなされ, これによって, 顎運動と咬合面形態との関係が明らかにされつつある. しかし, これらの研究のほとんどは, 正常咬合者についてのものであり, 顎運動と咬合面形態は密接に関係していることが示されたが, III級不正咬合者についてはそれらの点で今だ不明な点が多い. III級不正咬合者では, 被蓋の改善により咬合様式に大きな変化が現れるため, 当然顎機能にも影響が及ぶと考えられる.

Title : Study on the Occlusal Form in Angle Class III Malocclusion

Abstract : [Abstract]: When treating Angle's Class III malocclusions, it is very important that the occlusion is in harmony with the temporomandibular joint. Hence, it is vital to understand the occlusion of patients who have Angle's Class III malocclusion. Occlusal guidance was analyzed on mounted casts of 58 Japanese subjects (mean age 24 years and 1 month) with normal occlusion and 27 patients (mean age 17 years and 3 months) with Class III molar relations. Inclinations of occlusal guidance were measured on the mounted casts with reference to the axisorbital plane (AOP), in accordance with the principles of sequential guidance with canine dominance. Occlusal guidance on maxillary teeth was defined as the functional path between maximum intercuspation and the most eccentric occlusal contact immediately before disclusion. Excursions from the starting point to end point of occlusal guidance for protrusive and lateral excursions on maxillary teeth from the central incisors to the second molars were designated F1 and F2. The inclination of a line connecting F1 and F2 to the AOP was measured for each tooth. Wear facets on maxillary teeth were marked with two points. The coordinates of both points of occlusal guidance and wear facet were recorded with a 3D-digitizer. Angular differences between occlusal guidance and wear facets for AOP of subjects exhibiting an Angle's Class III relation were compared with the same angles on subjects with normal occlusion. Both guidance and wear facets of Class III cases showed a sequence from anterior to posterior teeth. The angles of guidance of normal occlusion and Class III malocclusion were significantly correlated in lateral incisors, canines, premolars and molars. Differences between guidance of occlusion in normal and Class III cases were found in the first' premolars and the first molars. Guidance on the first premolar in Class III cases was found to be similar to guidance on the canine in normal occlusion. The occlusal form in Class III malocclusion was similar to that of canine dominance guidance, but in the Class III cases, lateral guidance was on the first premolars rather than the canines. For that reason, the canine dominance is necessary for stabilizing occlusion after treating Class III malocclusion.
