Abstract : 「緒言」近年, 高齢社会において義歯の需要は増加すると予測され, 義歯とそれを取巻く口腔諸組織の形態的変化や生理的変化が注目されている. 加齢に伴う種々な口腔の組織変化の中で, 歯槽骨の変化は装着された義歯の適合状態に影響を与える可能性が大きく, 適合状態の低下は口腔組織に悪影響をもたらす結果となる. 義歯不適合による咬合圧の不均等な配分は, 義歯側では不均等な応力の発生や過大な応力の発生を招き, ひいては義歯破折の原因ともなる. 顎堤側では褥瘡性潰瘍の発生や歯槽骨の吸収を進行させる原因ともなる. 義歯床の適合性の影響を客観的に検討する方法の一つとして, 義歯及び床下組織に加わる応力分布の測定がある. 新倉は総義歯の変形や破折現象をストレインゲージ法を活用して解明し, 前田らは上顎義歯について有限要素法を用いた応力解析を行い, 最近では骨吸収のシミュレーションを行っている. 同じく有限要素法を用いた河野らは粘弾性解析を応用し, また本橋は補綴実習用顎模型をもとに作製した3次元モデルを用いて床下組織の応力分布の解析を行っている.

Title : Relationships between Alveolar Bone Resorption and Stress on Denture-Supporting Tissues

Abstract : [Abstract]: Changes in stress distribution with progress in resorption of the residual ridge were investigated on the basal surface of the denture and the cross section of the denture-supporting tissues before and after relining. Experiments were conducted to study these changes on a simulation model of a lower full denture. 1. When an external force was applied to the denture at an angle of 0, while the alveolar bone was being scraped away, stress tended to increase in both the buccal and lingual marginal portions of the denture base. On impact at an angle of 30 from the buccal side, stress decreased in the part of the denture base corresponding to the alveolar ridge, and increased in the lingual marginal portion of the denture base, in the part of the denture base supported by the residual ridge. 2. When an external force was applied to the denture at an angle of 0, or at an angle of 30 while the alveolar bone was being scraped away, the portion of the denture-supporting tissues corresponding to the lingual marginal portion of the denture base was more vulnerable to stress than when the denture was secured in position. This tendency was more remarkable when an impact was applied at an angle of 30 than at an angle of 0. 3. After relining, stress toward the lingual marginal portion of the denture base had a wide distribution throughout the basal surface of the denture. In the residual ridge area, stress distribution was displayed widely and evenly. This tendency was more remarkable when an impact was applied at an angle of 30, than at an angle of 0. These results suggest that both portions of the occlusal stress burden of margin and the denture-supporting tissues of the denture base are increased with lateral movements. In particular, with the failure of denture adaptation, the improvement of dispersion of the local stress concentration is secured by relining.
