| Abstract ： | 「緒言」遊離端欠損を含む多数歯欠損症例は, 残存歯および欠損部顎堤に支持を求めなければならない. 歯と顎堤の被圧変位量の差は大きく, 遊離端欠損の義歯設計時には支台歯および顎堤にかかる負担差を各々考慮し, 長期にわたり, その機能を持続させなければならない. 支台・歯にかかる負担をより少なくするために, 設計(形態的)条件, 支台装置(緩圧性, 非緩圧性)の選択, 印象法など多くの方法が論じられ, 検討されてきた. Shohet, MatsumotoらおよびCecconiら等は, クラスプを直接支台装置と設計した支台歯の負担力量について調査をおこない, 間接支台装置の配置による負担状態の変化が大きく, 欠損部顎堤の形態における負担力量の差異は小さかったと模型実験において報告している. Fennerら, 松元ら, 田口らは, 遊離端義歯における緩圧性支台装置の検討, およびクラスプを用い局部床義歯の咬合時における支台歯の動揺を口腔内実験を行い, クラスプを設置した支台歯は装着時動揺度が減少するが, 一週間後では約2倍の動揺度の増加を示したと報告している. |

| Abstract ： | [Abstract]: By the cases of multiple missing free-end, they require the support for residual teeth and the saddle. The pressed malposition ratio of the saddle is bigger than the teeth, corsider the differences of them, restore the oral function and maintain them in a good condition. It have been researching the design of denture, choices of retainers and impression methods to reduce the load of abutment teeth. In this case, connected the left lower lateral incisor, canine and first premolar and designed a partial denture with the non stress-breaking attachment. After a few years past, observed bow the denture functioned in the mouth by masseters electromyogram and the result were as follows. 1. When there were no occulusion of posterior teeth, the masseter activity ration were low at the time of tapping. 2. Connected the teeth of paradental diseases and designed as retainer in one body of a denture, the teeth could be maintained over an extend period of time. 3. About one year later after the insertion, there were gingival hyperplasia under the female of the attachment. |
