Abstract : 「緒言」エナメル質に対するリン酸処理法が確立して以来, 接着歯学における研究の対象は象牙質に向けられ, より優れた強固な象牙質接着システムの開発が望まれている. しかしながら, 象牙質は構造上その組成から湿潤度が高く, 多くの有機質成分から構成されているため, 疎水性レジンの接着を極めて困難にしている. 従来の象牙質接着は, エナメル質と同時に酸処理を施すことにより得られていたが, dentinal plugの除去による歯髄為害作用や歯髄内液の流出による接着性の阻害, あるいは酸処理によるコラーゲンの変性などさまざまな問題が報告されており, 現在では, 透過性を抑制し外来刺激の防御と成り得るdentinal plugは除去せず, 表面の改質を目指す表面処理剤や, 接着を向上させるさまざまなボンディング剤や象牙質プライマーの開発が行われている. このような象牙質に対する接着性能の向上により長期の臨床成績において良好な予後が期待されるが, 接着初期に得られる高い接着力も修復直後から加わる咬合力や温熱的ストレスあるいは水の影響等により, 経時的に低下することが懸念される.

Title : Adhesive Fatigue Characteristics of Various Dentin Adhesive Systems

Abstract : [Abstract]: Many experiments about the effectiveness of dentin adhesive system have been carried out to improve the adhision of resin composites by measuring the shear and tensile bonding strength. However, the tooth-composite interface is subjected to repeated mechanical stress in the oral cavity, therfore, the adhesive fatigue characteristic is the most appropriate parameter to evaluate a adhisive ability of the bonding system. The purpose of this study was to determine the shear bond strength and fatigue resistance of four dentin adhesive systems, Clearfil Photo Bond (Kuraray), experimental RTS-200 (Kuraray), Scotchbond 2 (3M) and Tripton (ICI). Photo Bond is a phosphate-based bonding system, other systems are a primer-conditioning system. Each adhesive system were applied to a flat dentin surface of bovine tooth in accordance with the manufacture's instruction. Bonded specimens were stored in a 37℃ deionized water for 1 day, then were subjected to the shear bond or fatigue test. The fatigue tests were carried out using a fatigue testing device until the failure occurred or the number of stress cycles reached to 105 cycles, with repeated stress of 4Hz in deionized water at 37℃. The obtained results were as follows. 1) RTS-200 system provided the highest shear bond strength of 12MPa and adhesion values ranging from 7 to 9 MPa were found for Photo Bond, Scotchbond 2 and Tripton, and no statistical differences could be found among them. 2) The order of magnitude of the adhesive fatigue resistance is RTS-200 system/Scotchbond 2/Tripton/Photo Bond. The fatigue resistance of Photo Bond was cleary the lowest. 3) An adhesion of Photo Bond suffered a great fatigue influence at 60% load level of the bond strength. While, those of Scotchbond 2 and Tripton received a fatigue influence at 90% load level of each bond strength. Significant correlations were not indicated between the shear bond strenth and the fatigue endurance for dentin adhesive systems used in this study It was suggested that the evaluation of an adhesive durability is a better method to estimate the efficacy of dentin bonding system in the oral cavity and that the application of a primer to dentin enhance the adhesive fatigue resistance of the dentin adhesive system.
