Abstract : 「緒言」臨床上, 下顎多数歯欠損の遊離端症例に対するパーシャルデンチャーは, 構成要素としての生体側と義歯側の両面において多くの問題を抱えているので設計に困難が伴う事が多い. 生体側の主な問題点としては, 残存歯と欠損粘膜の被圧変位量差, 床下粘膜間の被圧変位量差, 残存歯の挺出や動揺ならびに残存歯周囲の歯槽骨吸収に歯冠歯根比の増大等が挙げられる. また義歯側の主な問題点としては, 機能時の義歯床の変位, 義歯床破折ならびに残存歯に左右される困難な人工歯排列等が挙げられる. この様な問題の解決に対し, Stud attachmentやBar attachmentの概念を総義歯に取入れたOver dentureが多く利用される様になり, 同時に多くの研究者により研究結果が報告されてきた. 生体側に関する研究では, 支台歯の形態や保護法, 機能時の支台歯の変位量, Over dentureを取り巻く衛生的諸問題の対処法など, また義歯側に関する研究ではOver dentureの振動解析, 支台歯を含む義歯の動態や変位量など, 種々な支台装置の特色や機能解析などについて報告されている.

Title : The Stress Analysis of Denture Base on Overdenture Retainer

Abstract : [Abstract]: We manufactured a full denture and the three types of over dentures using a coping, Ceka attachment and Dolder bar attachment. We also manufactured the two types of dentures using a soft relining material on the coping model over denture for occlusal buffer purposes. (One that the coping contacts with the soft relining material and the other, non-contact type). The non-contact dynamic stress analyzer was used for the measurement. We conducted a stress analysis of the denture base surface and computed specific stress value on the occupying area. The results were as follows 1. When the over denture was installed with a retainer as the fulcrum for vertical or horizontal distal revolution, the concentration of the stress was percieved at the retainer installation portion, the end margin portion of the denture base and the alveolar crestline portion. 2. The stress production on the denture surface was little on the over denture with the soft relining material and the stress production on the non-contact type (the coping and the soft relining material) was very little. 3. The most stress diminution was found on the over denture with the stress reducing Ceka 721 which was tendency to function against vertical stress. 4. In case that the denture base was tendency to subside, it was observed that the stress concentrates at the frenulum of the lower lip and the frenulum of the cheek.
