Abstract : 「緒言」 シスプラチン(CDDP)を主体とするCF療法は抗腫瘍スペクトルが広く, 口腔扁平上皮癌のadjuvant chemotherapyとして有効であることが報告されている. 自治医科大学歯科口腔外科でも1986年から口腔扁平上皮癌の患者に対しCF療法を施行した結果, 奏効率が80%であることなど, その有効性を報告してきた. しかしCF療法による治療効果をさらに向上させるためには, 治療効果をある程度予測することが必要と考えられる. 抗癌剤の感受性試験の開発については既に多くの施設で研究が進められているが, 従来の感受性試験では判定までに長時間を要することや判定結果と従来の治療効果が必ずしも一致しないことなどから, 一般に普及するまでには至っていない. 一方, CDDPはS期の細胞に作用してその抗腫瘍効果を発揮することから, 腫瘍組織の増殖能がCDDPの抗腫瘍効果に影響を与えることが推測される.

Title : The Immunohistochemical Study of Relationship between Therapeutic Effects by CF Therapy and Cell Proliferating Activity in Oral Squamous Cell Carcinoma.

Abstract : Abstract : Neoadjuvant chemotherapy has been extensively investigated in head and neck cancer, and recent studies show high overall response rates to chemotherapy. However, there are some variations in responsivility to chemotherapeutic drugs depending on each case. In order to clarify the correlation between therapeutic effects of neoadjuvant chemotherapy and cell proliferating activity, the effects of neoadjuvant chemotherapy with CDDP and 5FU (CF therapy) and PCNA positive rates on oral squamous cell carcinoma were examined. The patients analyzed were 33 cases of oral squamous cell carcinoma and the patients had been treated with neoadjuvant chemotherapy with CDDP and 5FU, the tumor cell proliferating activity was assessed by PCNA immunohistochemical staining during 7 years from 1986 to 1992 at the Department of Dentistry and Oral surgery, Jichi Medical School. The average age of the patients was 61.6 years, and 23 patients were male and 10 patients were female. The primary sites of the tumors were the tongue in 13 patients, the mandibular gingiva in 11, the buccal mucosa in 3, the floor of mouth in 2 and the lower lip in 1. One patient was treated with 1 couse of CF therapy, 5 patients with 2 couses, 15 patients with 3 couses, 10 patients with 4 couses, and 2 patients with 5 couses. After these treatments, therapeutic effects were determined as follows : 7 patients were complete response (CR), 16 patients partial response, (PR), 9 patients no change (NC) and 1 patient progressive desease (PD). HE staining and PCNA staining by SAB methods using anti-PCNA antibody were performed. Positive rates of tumor cells were counted in randomly selected 10 areas per one specimen by Olympus microvideometer. There were no statistical relationship between PCNA positive rates and clinical findings such as TNM classification and stage grouping. The average of PCNA positive rates in CR group was 41.5+-9.4%, those in PR group and NC+PD group were 29.0+-15.2% and 23.2+-7.2%, respectively. The PCNA positive rate in CR group was significantly higher than that in NC+PR group (P<0.001) ; the PCNA rate in CR+PR group was also much higher than that in NC+PD group (P<0.01). These results show that cell proliferating activity assessed by PCNA staining must be a good parameter which predict therapeutic effects of neoadjuvant chemotherapy with CDDP and 5 FU in oral squamous cell carcinoma.
