Abstract : 「緒言」 近年, ヒドロキシアパタイトやチタンなどの骨親和性材料の応用と手術法の改良により, 人工歯根の臨床成績が向上し, 使用頻度も増加しつつある. しかし, これらの人工歯根はいずれも骨と直接結合することを目指したものであり, 天然歯の歯根膜がもつ緩圧機能を欠くために, 衝撃に対して著しく高い応力を招き易く, 頻回の衝撃により骨や人工歯根の疲労破壊を生じさせる危険性があること, また可動性に乏しく咬合時の隣在歯の沈下や天然歯と連結した場合に機能圧の集中を招きやすいこと, さらに歯根膜中の圧受容体の欠如が, これらを一層増悪させるなど, 様々な問題点が生じている. 最近, 人工歯根の内部に緩衝構造を設けることによって歯根膜の力学的機能を代償しようとする試みもなされているが, 歯根膜の持つ生理学的機能をも代償することは不可能である. このことから歯根膜をもつ人工歯根の必要性が再認識されるとともに人工歯根表面に歯根膜を再生させるハイブリッド型人工歯根の試みがなされている.

Title : Study on the Regeneration of Periodontal Ligament around Dental Implants - Effect of Dog Periodontal Ligament Fibroblast Conditioned Medium (DPLF-CM) on Proliferation and Differentiation of Periodontal Ligament Cells -

Abstract : [Abstract] In regenerating periodontal ligament (PDL) around the roots of artificial teeth after implantation of cultured PDL cells onto the collagen-coated root surface, an important role is played by some physiologically active substance that promotes adhesion of the cells both to the teeth and the alveolar bone and induces cell proliferation and differentiation. To obtain such an activator, the supernatant of the conditioned medium (CM) of dog periodontal ligament fibroblast (DPLF) was fractionated with the IEC-DEAE column. Using hydrophobic dishes coated with the each fraction, we investigated cell proliferative activity (DNA content) and ALPase activity as well as electron microscopic feature of the contact surface between the cultured cells and the dish. 1. The DPLF-CM could be separated by IEC-DEAE column into six fractions. 2. Every fractions promoted increase in DNA content and ALPase activity of the cultured DPLF. Among these, remarkable augmentation of DNA content and ALPase activity were observed in fractions 2 and 3. DPLF cultured with fraction 6 showed disproportionately high level of ALPase activity compared with the proliferative activity. 3. SDS-PAGE analyses showed the presence of various forms of secretary proteins in each fraction. Two types of proteins at estimated molecular weights of 210 KDa and 160 KDa in fraction 2 and one protein at an estimated molecular weight of 25 KDa in fraction 3 were revealed to the type of proteins which were not found in the other fractions. 4. Electron microscopic analyses showed that DPLF cultured in the dish coated with each fraction were in close contact with the surface of the dishes and that fine fibers which protruding from the DPLF clinged to the dishes. This observation was particularly noteworthy in the dishes coated with fractions 2 or 3. On the contrary, a wide gap between DPLF and the dishes was observed in the control group. These findings suggest that the DPLF-CM contain specific physiological factors that induce proliferation and differentiation as well as cell adhesion of the DPLF.
