Abstract : 「はじめに」 種々なう蝕予防手技に関する多くの研究から, フッ素(以下, “F”とする)の応用は, その効果ならびに実用性の点で優れていることが明らかとなった. わが国において, Fの応用は全身応用よりも局所応用が広く利用されている. このF局所応用法にも種々あるが, 歯磨き行動が習慣化した現代においてF配合歯磨剤の使用は日常簡単に利用できる方法として位置づけられている. しかしながら, 現代の我が国のF配合歯磨剤の市場シェアは約46%(1994年)であり, 90%以上を示している諸外国に比べ低率である. F配合歯磨剤を使用することで局所的にFが適用され, 低濃度のFが口腔環境中に保持される. このFによってエナメル質の成熟現象ならびに初期う蝕病巣の再石灰化の促進と, 歯垢中での酸産生の抑制効果が期待できるのである. このようなF配合歯磨剤のう蝕予防メカニズムから, 実用的でより効果の高い使用方法を考えるには, 安全で, しかも歯磨き(以下, ブラッシングとする)そのものの物埋的効果を阻害せずに, できるだけ多くのFを, できるだけ長時間口腔環境中に保持させるにはどうしたらよいかという点も検討すべきである.

Title : Effect of Mouthrinse on Fluoride Retention in Saliva Following Toothbrushing with a Fluoridated Dentifrice

Abstract : [Abstract] Use of fluoride dentifrice is an established method for prevention of dental caries which can easily be performed in regions where tooth-brushing is a custom. Its mechanism of action is that residual fluoride ions present in the mouth after the use of fluoride dentifrice promote remineralization in incipient dental caries and inhibit acid production in dental plaques. To enhance the retention of fluoride ions in the mouth above the effective concentration for a prolonged period is important. The author considered the conditions of mouthrinsing after tooth-brushing with fluoride dentifrice to be important factor influencing the retention of fluoride ions in the mouth. In the present study, the author examined the effect of the conditions of mouthrinsing on the residual fluoride concentrations. Three variables ; the amount of water (ml), the time of mouth-rinsing (sec), and the number of times of mouth-rinsing, were the items considered. The fluoride content in saliva was taken as a measure of the residual fluoride concentration in the mouth after the use of a fluoride dentifrice. The author found that the number of times of mouthrinsing affects the fluoride content in saliva most. Therefore the author examined the changes in the fluoride content in saliva over a period after tooth-brushing with fluoride dentifrice followed by various times of mouth-rinsing. The results obtained are summarized as follows : 1. The degree of fluoride retention in the mouth is influenced by the secretion rate of 1 saliva and the number of times of mouth-rinsing. 2. After tooth-brushing for 2 min with 0.8 g fluoride (950 ppmF) followed by mouthrinsing one to five times with 12.5 ml water for 2 sec each time, the fluoride level in saliva base-line level within 4 to 10 hours. 3. The residual fluoride level in saliva remained over 0.05 ppm for 1 hour after tooth-brushing and 4 times mouth-rinsing even in individuals with high clearance of fluoride due to a high secretion rate of saliva. 4. To retain a high fluoride level during sleep, it is advisable to brush the teeth with fluoride dentifrice before going to bed and the number of times of mouth-rinsing should be kept less than 4 times. 5. The fluoride level in saliva was kept high by tooth-brushing with fluoride dentifrice three times a day. 6. There is no anxiety for side-effects due to excessive intake of fluoride by reducing the number of mouth-rinsing of three or less.
