Abstract : | 「緒言」 失われた口腔機能を再構成するために補綴装置が用いられるが, その装置の使用期間を次のように定義付けることができる. それは, 患者が口腔内装着後の新義歯に慣れて望んだ機能を発揮するまでの期間, 安定した機能が発揮される期間, 並びに何らかの違和感を感じるが使用可能な期間の3期である. そして, 口腔諸組織の一部となって安定した機能が発揮される期間は決して長いとは言えず, パーシャルデンチャーの平均使用年数は約5年と報告されている. また, 義歯更新の原因は, 義歯構成要素の破損並びに義歯床や支台装置の不適合による維持力低下が多いという報告がある. 特に支台装置については, 破損や維持力の低下を防ぐようなアタッチメント形式のものが検討されてきたが, 依然として臨床ではAkers claspの使用頻度が高く, 維持力や形態などについて多くの研究がなされている. Akers claspは支台歯との摩擦抵抗や支台歯のアンダーカットを利用することによって義歯の支持, 維持並びに安定を発現するが, 義歯の長期使用によって着脱が繰り返され, 維持力が低下するとされている. |

Abstract : | [Abstract] The abrasion of retainers may be given as one of the causes of failure in denture-retaining force. Conceivable factors leading to the failure include abrasion and deformation of clasps, and attrition of the surface of clasped teeth. In the present study, we examined time-cource of changes in the degrees of attrition, retentive force and deformation, using various combinations of clasp and crown materials. The materials used for clasps were Pt-Au, Co-Cr, and titanium. For crowns, enamel, porcelain and Pt-Au were used. Each combination of clasps and crown was inserted and removed by means of a special device 10,000 times. Retentive force and distance between clasp tips were measured every 1,000 times, and the rubbed surface was observed under a scanning electron microscope (SEM) every 2,000 times. Measurment of denture-retaining force prior to the insertion-removal cycle showed that the Co-Cr clasps were strongest ammong the materials used, irrespective of the type of crown. In every combination, retentive force fell off sharply at an early stage of testing cycle (2,000~4,000 times) on the average, but thereafter the decreasing tendency was not remarkable. Nonetheless, every type of clasp attached to the porcelain crown showed a strong tendency to decline in retentive force. Measurement of permanent deformation revealed that the titanium clasp undergoes the largest change. The Co-Cr clasp changed in shape as large as the titanium clasp at an early stage of testing cycle, but later the occurrence of deformation leveled off. The Pt-Au clasp continued to change throughout testing time-cource. SEM observation demonstrated that the porcelain crown gives rise to a great deal of abrasion in the Pt-Au and titanium clasps, and revealed that the Co-Cr clasp causes the Pt-Au crown to wear away considerably. |
