Abstract : 「緒言」 歯科領域にCAD/CAM (Computer Aided Design/Computer Aided Manufacturing)が導入されたのは, 1970年代後半からである. 神奈川歯科大学補綴学教室第III講座においても, 1979年藤田の下顎運動検出機構の報告を始めとし, 6自由度下顎運動計測の開発研究を進める一方, 1984年に数値制御による補綴物製作の可能性を報告して以来, 6自由度下顎運動と歯列形状を応用したCAD/CAMの研究開発を行ってきた. 近年, 歯科CAD/CAMシステムも市販されるようになり, 徐々に実用化に近づいている. しかし, 歯冠形態の再現方法, 製作物の適合精度, 製作効率, コストなどの問題から, 臨床に広く応用されるには至っていない. 中でも製作効率と適合精度とは相反する関係にあり, より合理的なシステムの検討が必要とされているのが現状である.

Title : A Study on Design and Fit of Crown Interior by CAD/CAM

Abstract : [Abstract] Methods for designing and processing crown by means of computer-aided design and computer-aided manufacturing (CAD/CAM) have advanced in recent years. However, further improvements of both software and hardware of CAD/CAM are required to obtain more accurate measurement of the abutment and fabrication of the crown. In the present study, I developed and improved software for measuring the abutment and for milling of the interior of the crown. By using this new CAD/CAM system, I tested the system to see how accurately it reproduced the crowns and also examined fitness of the crowns. Automatic determination of the marginal line of the abutment was performed by a single-ended cylindrical probe. Measurement was done by utilizing both polar and cylindrical coordinates at 5,040 points over the abutment. A ball-ended mill with a diameter of 2 mm was employed for the milling of the crown. The milling was programmed so as to start cutting from the margin and stop at the middle of the occlusal surface at one stroke. A program which could change the thickness of the cement space between the interior of the crown and abutment was also developed. Reproducibility and fitness of the crown with different cement space (60, 80, and 100 μm) were examined by using black silicone technique. The standard deviations of the mean thickness of the silicone between the crown and the abutment at different parts did not exceed the machine-margin of error (10 μm), suggesting that the system had high reproducibility. The cement space with 60 μm thickness significantly affected the fitness of the crown on the abutment when compared with those with 80 μm or 100 μm. From these results, it is considered that new CAD/CAM system has sufficient reproducibility and accuracy for the fabrication of the crown. This also has an advantage to control a cement space at the interior of the crown, hence obtaining good fitness.
