Abstract : 「緒言」 骨格性不正咬合の咬合治療においては, 矯正治療のみでは満足な改善が得られず, 外科的処置を併用しなければならない場合が少なくない. また, 近年, 患者の側からも, 咬合の安定のみならず顔貌の調和を求める声が高まってきている. 顔は人の個性を表す最も重要な要素であり, 顔貌の改善が果たす心理的, 社会的意味は大きい. それゆえに, 治療計画の立案に関しては, 咬合のみならず顔貌の改善に関しても注意がはらわれなければならない. しかし, 実際には顔貌の変化に関する術後の予測が困難であるため, 計画通り治療が行われたにもかかわらず, 十分な改善を得ることができない場合がある. 本研究は, 顔面非対称患者の治療に外科的矯正治療を適用する際, 手術計画の立案並びに治療に関して考慮すべき点について検討することを目的とした. 「資料ならび方法」 本研究では, 神奈川歯科大学付属病院矯正科において矯正治療を行った先天異常を伴わない骨格性顔面非対称症例のうち, 外科手術を併用した男子7例, 女子3例の計10例を対象とした.

Title : Assessment of Treatment Result of Orthognathic Surgical Orthodontic Cases with Facial Asymmetry

Abstract : [Abstract] In order to improve the facial disharmony of skeletal malocclusion, orthognathic surgery has been incorporated into the total reconstruction of occlusion in addition to orthodontic treatment. To obtain a successful result, orthognathic surgical orthodontic treatment in the patients with facial asymmetry must be directed to stabilization of the corrected mandibular position which is thought to be correlated with the cant of maxillary occlusal plane. The purpose of this study is to evaluate the relationship between the occlusal plane change and improvement of facial asymmetry using the methods of orthognathic surgical orthodontics. Pretreatment assessments of facial asymmetry indicated that the degree of facial asymmetry is primarily related to the lateral shift of the mandible. The cases in which complete facial symmetry was not obtained indicated that the maxillary occlusal plane remained in oblique inclination in frontal view of the head film, hence an index of mandibular lateral displacement (L 3 to L 0) remained higher than in successful cases. There was close correlation between the degree of mandibular lateral displacement and the inclination of maxillary occlusal plane in post-treatment facial morphology. Some cases indicated lack of success was caused by the inconsistency of the maxillary dental mid-line with a facial mid-line. These findings suggested that it is very important to make deliberate planning of orthognathic surgery when the patients have strong inclination of maxillary occlusal plane and/or mid-line discrepancy of maxillary dentition.
