Abstract : 「はじめに」 昭和39年(1964年)4月に神奈川歯科大学附属病院歯科矯正科が開設されて以来30余年が経過した. この間の一般社会情勢は大きく変化した. 特にここ十数年間の政治・経済は激動の世代であった. 例えば, 与野党の交代, 高度経済成長の不安定さ, バブル景気, そして崩壊, 金融機関の破綻, 少子化現象, 高齢化社会, 歯科矯正の分野においても, 矯正科の標榜認可, 唇顎口蓋裂疾患・顎変形症の保険導入等々あり, 今後益々複雑化していきそうである. また, 昭和45年(1970年)に厚生省は, 人口10万人に対して, 歯科医50人を目標とすると発表したが, すでに昭和55年(1980年)には達成された. そして, それが今日では歯科医師数の過剰問題として発展してきた. このように矯正治療をとりまく環境が大きく変化してきた. そこで, 来院患者の実態を把握することは, 矯正臨床上, 極めて有意義なことと考え, 今後の外来運営の参考とする為に, 最近4年間とその10年前の動態を調査した.

Title : The Tendencies of Orthodontic Patients in Kanagawa Dental College Hospital

Abstract : [Abstract] Orthodontic clinic in Kanagawa dental college hospital established in 1964 and 34 years had passed since this time. This report concerns mairly the result of clinicial observations in the last 15 years (April 1983 to March 1987 ; A group and April 1993 to March 1997 ; B group). Materials were analyzed for constitution of the patients on several reservations. 1. Total number of patients who had visited the clinic was 1,464 (A group) and 1,089 (B group). 2. The number of patients who has visited the clinic was more than 300 every years in A group. These showed a tendency to decrease in B group. The ratio between males and females was 1 : 2. 3. Regional distribution of patients showed that the greater part of the patients visited from Yokosuka (A group ; 40.1%, B group ; 43.5%) and Yokohama (A group ; 32.5%, B group ; 26.5%) and Kamakura (A group ; 6.5%, B group ; 4.4%). 4. The treated patients were centered on primary school age in A group (66.5%) . The most centralized patient's age was more than 19 years old in B group (37.3%). 5. Showing with Hellman's dental age, IIIB (37.7%) and IVC (21.3%) in A group, and IVA (45.2%), IIIB (23.7%) in B group. 6. The frequency of the type of malocclusions was high in the order of Cl I (45.4%) , Cl II (31.6%) and Cl III malocclusion (22.5%) in B group. The incidence of the malocclusions was the highest in Cl I malocclusion (45.4%) in A group. 7. The incidence of malocclusions was the highest in the anterior cross bite (29.2%) and in crowding of the teeth (24.4%) in A group, but was the highest in crowding of the teeth (32.0%) and in the anterior cross bite (19.9%) in B group.
