Abstract : 「緒言」 歯と歯槽骨の不調和(discrepancy)により生じる叢生は, 増加傾向にあるとされている. このdiscrepancyの原因として考えられる要因は, 歯が大きく顎骨が小さい場合, あるいは, 顎骨が正常でありながら何らかの要因により萌出場所が不足している場合などが考えられる. Nakaharaらは, アジア圏6ヵ国のなかで, 比較的民族性の保持されている現代人の第三大臼歯の萌出率や欠如の状態を調査した結果, 日本人の第三大臼歯は他の人種にくらべて完全に萌出している症例が少なく, タイやフィリピンの人種では完全に萌出している症例が大部分を占めていることを報告をしている. 日本人の下顎第三大臼歯は, 萌出場所の不足から完全に萌出することが困難であり, 埋伏症例や半埋伏症例の増加傾向がみられる. また, 第三大臼歯の発育は歯冠の完成時期は12~16年であり, 萌出時期が17~21年と他の歯種に比べ比較的長期であるため, 矯正治療の保定期間中に第三大臼歯が原因となり歯列の安定を損ねることから, 第三大臼歯の歯根形成期には定期的なエックス線による診査が必要であると報告している.

Title : Eruption State of the Mandibular Third Molar and Mandibular Morphology in Different Races -A Comparison between Japanese and Non-Mongolian-

Abstract : [Abstract] The objective of this study was to elucidate whether there is a difference between two races, Japanese and Non-Mongolians (dry skulls) in the morphology of the mandible, which affects the eruption of the lower third molars. 1. The mandible in Japanese were generally larger in subjects with fully erupted third molars, allowing ample room for the eruption of a the mandibular third molar in the posterior region. These cases were characterized by the opening of the mandible accompanied by the elongation of the first molars. 2. Distance measurements revealed that the mandibles of Non-Mongolians were generally smaller than those of Japanese. However, a relatively large proportion of the length of the mandibular body to the length of the mandible in Non-Mongolians suggested that ample space in mandibular body is essential for the eruption of the lower third molar. When this proportion is small, third molars tend to become impacted since ample space for the eruption of a third molar can not be secured in the mandibular body. The degree of overeruption in the posterior region was smaller in Non-Mongolians. 3. The crown width in Non-Mongolians was significantly smaller than that in Japanese. However, since there was no significant difference in the crown width among Japanese, it was suggested that in Japanese, conditions of eruption of third molars are largely affected by the location of eruption in the mandible.
