Abstract : 「緒言」 Guided tissue regeneration (GTR)はMillipore filterやe-PTFE (expanded-polytetrafluoloethylene)などの人工素材でできたメンブレンをもちいて新付着を獲得する歯周治療法として始められた. GTRに関する研究が進むにつれて, メンブレンを使用することで歯肉上皮および結合組織と歯槽骨の細胞の環境が隔離された場合, 新付着のみならず歯槽骨再生が行われることがわかってきた. 特に歯槽骨を増生させる方法をGuided bone regeneration (GBR)と呼び, GTRと区別されるようになってきた. 一方, GBRを用いることで, 幅径や高径の少ない歯槽骨にもimplantを埋入することが可能となってきた. 歯槽骨に吸収添加が起こるとき, 歯周組織の微小循環が関連していることが近年の研究で解明されてきた.

Title : Morphological Changes in Alveolar Bone Regeneration and Vascularization during Healing of Extraction Socket Using PTFE Membrane

Abstract : [Abstract] Guided bone regeneration (GBR) is a periodontal treatment to restore the loss of alveolar bone. In the present study, nonresorbable Polytetrafluoroethylene (PTFE) membrane was applied to the extraction socket and vascular resin cast model of its region in dog was examined under scanning electron microscope (SEM) 14, 30, and 60 days after the surgery to assess the effect of the PTFE membrane. Fourteen days following surgery : The extraction socket was filled with newly formed porous bone. Bone aposition was observed upward and sideways beyond the alveolar crest. Regenerated blood vessels were distributed in the newly formed porous bone. Thirty days following surgery : In the center portion of the extraction socket, a dense, newly-formed vascular network and regenerated bone trabeculae were observed. Newly-formed bone was regenerated into compact bone beneath the PTFE membrane. A flat vascular network from the periosteum was observed on the PTFE membrane. Sixty days following surgery : Compact bone was observed beneath the PTFE membrane and the number of blood vessels in bony tissue was decreased. In spongy bone, regenerated bone trabeculae and a vascular network from the alveolar bone marrow were observed. These findings suggest that GBR technique may be highly effective on alveolar bone regeneration. It can be noted that height of the newly generated bone can be artificially regulated by positioning the membrane above the alveolar crest. It is also postulated that vascular regeneration dynamics may modify the efficacy of GBR.
