Abstract : 上下顎骨内に発生する歯原性嚢胞は, 歯を形成する原基であるエナメル器や歯の発生初期に顎骨内に陥入した原始口腔上皮に由来する歯堤あるいは, これらの顎骨内遺残上皮成分が種々の原因で嚢胞化すると考えられている. しかし具体的な嚢胞形成メカニズムは現在明らかにされていない. 一方, 最近になって細胞死(アポトーシス)抑制機能をもつbcl-2がヒト歯胚の発生初期から発現し, 歯の萌出後顎骨内に残存するMalassezの遺残上皮をはじめとした歯原性上皮にBcl-2タンパクの発現が認められることが報告されている1~4). この歯原性上皮に発現するbcl-2のアポトーシス抑制機能が遺残の原因となる可能性が示された. さらに, 多くのエナメル上皮腫の腫瘍細胞, エナメル上皮線維腫, エナメル上皮線維歯牙腫, 歯原性歯肉上皮過誤腫などの歯原性腫瘍の上皮成分に特異的なBcl-2タンパクの発現が観察され3,4), このBcl-2タンパクの発現がみられた腫瘍組織からRT-PCR法でbcl-2mRNAのメッセージが認められることも報告されている4).

Title : Expression and Role of Bcl-2 on Odontogenic Cystic Lesion Pathogenesis

Abstract : Recently, it has been reported that the expression of bcl-2 was observed in human tooth germ or odontogenic tumor and cyst, indicating the possibility that the anti-apoptotic effect of bcl-2 may be involved in the pathogenesis of these lesions. In this study, the expression of Bcl-2 protein in the epithelium of cyst wall was examined using an immunohistochemical reaction in 36 patients with primordial cysts, 10 with dentigerous cysts, 12 with radicular cysts, 2 with calcifying odontogenic cysts, 5 with naso-palatine duct cysts and 10 with epidermal cysts occurring in the oral cavity and skin of head and neck regions. A part of odontogenic developmental cysts were examined for the detection of bcl-2 mRNA by the RT-PCR method, the rearrangement of bcl-2 genes by the Southern blot analysis and the telomerase activity by telomeric repeat amplication protocol (TRAP) assay using materials frozen in liquid nitrogen at isolation. As the result of immunohistochemical reaction, an intense expression of Bcl-2 protein was observed in the epithelium of cyst wall of 72. 2% of primordial cysts, especially 84. 6% of odontogenic keratocyst and 100% of calcifying odontogenic cyst. A weak expression of Bcl-2 protein was also observed in the epithelium of cyst wall of radicular cyst. There was no expression of Bcl-2 protein in the epithelium of cyst wall of naso-palatine duct cyst and epidermal cyst resulting from non-odontogenic epithelium. The RT-PCR method, a message of bcl-2 mRNA was detected in 4 of 5 patients with odontogenic keratocyst through the RT-PCR method. In the Southern blot analysis, no band of rearrangement of bcl-2 genes was detected in any primordial cyst. However, telomerase activity was detected in 2 of 7 patients with primordial cyst, including one of the keratotic type and one of the non-keratotic type. From the above results, the pathogenesis of odontogenic developmental cyst could be caused by inhibition of apoptosis associated with the expression of bcl-2. The expression of bcl-2 enhanced telomerase activity could lead to a high proliferation of a part of the epithelium of odontogenic developmental cyst wall.
