Abstract : 骨格的な下顎側方偏位症例の中で, 上顎咬合平面の傾斜に主因をもつ症例は, 上顎骨切り術を併用する上下顎同時移動術によって咬合平面の傾斜を改善し, 下顎の側方偏位の改善をめざす方法をとることが多い1). しかしモデルサージェリー上の治療目標にもとづいた上顎骨の3次元的な位置設定を, 実際に咬合器上で正確に再現するには術者の熟練が要求される. 特に上顎骨の前頭面での回転を伴うような設定では, スプリットキャストの基底面が完全に離開してしまうことや歯の移動量と基底面部での移動量が異なることなどから, 定量的に移動量の把握をするのは困難である. 咬合器上での上顎骨の移動にはいくつかの方法が報告されているが2), いずれも専用の機器が必要とされる. われわれは特別な機器を用いることなく, 従来のダブルスプリットキャスト法を改良して容易に上顎模型の移動を行い, 上下顎同時移動術を行ったので報告する. 方法および症例 1. 方法 1)上顎模型の付着とダブルスプリットキャストの作製 患者から採得した上顎模型をフェイスボウトランスファーを行って, SAM咬合器(SAMII,SAM社)に付着した.

Title : A Case of Mandibular Asymmetry Aligned by a Modified Double Split Cast Method in Orthognathic Surgery

Abstract : It is often difficult to treat adult patients who have significant skeletal severe asymmetry, without performing orthognathic surgery. This paper describes the treatment and morphological findings of a case of a mandibular asymmetry patient who underwent orthognathic surgery. Usually, therapeutical procedures are not started prior to establishing a 3-dimensional (3-D) treatment goal. Thus, it is important to understand the 3-D treatment goal. In response to this problem, we developed a classical double split cast method and have been using it in such orthognathic surgery, with favorable results. In this method, information of roentgenographic cephalometrics analysis was applied for classical double split cast method on the articulator. It is a modified double split cast method which has a possibility to easily monitor the treatment goal on the articulator. The patient was a 23-year-old female with severe mandibular asymmetry. Skeletal features included a mesio facial pattern. The patient underwent preoperative orthodontic treatment, orthognathic surgery, and postoperative orthodontic treatment. By correction of the unilateral posterior crossbite with orthognathic surgery, jaw deviation was improved simultaneously. Class I molar and canine occlusions were achieved, and then the ideal overjet and overbite were established. Morphological analyses after surgery indicated that both occlusion and the facial profile were remarkably improved by this treatment.
