Abstract : フッ化物(以下“F”とする)応用法は世界中で広く利用されている有効な齲蝕予防手段である. 中でも全身的応用法の一つである水道水フッ化物添加(以下“WF”とする)は, アメリカやカナダでは1945~1946年に始まり, アメリカの疾病予防センター(CDC)により20世紀の公衆衛生分野における10大業績の一つとして歯科関係で唯一挙げられるなど, 多大な評価を受けている. このように, WFはその高い齲蝕抑制率と安全性ならびに優れた費用便益率といった公衆衛生特性から, 現在では世界の60ヵ国に広がっている. 日本では過去において一時的に京都市の山科地区においてWFが実験的に行われたものの1), 現在のところ人工的に実施されている地域はない. しかし, 最近になり日本でも一部の地域の保健施策としてWFを導入する動きが具体化している. このようなFの全身的応用法を実施する上でFの有効性は勿論のこと, 日本人にとってのFの摂取や排泄についてのデータを蓄積し, 安全性の根拠を提供していく必要がある.

Title : Studies on the Balance of Fluoride Intake and Excretion
Subtitle : Influence of 1 mg or 2 mg Fluoride Burden on Urinary Fluoride Excretion

Abstract : It is necessary to establish the safety base of 1mg fluoride supplementation through the accumulation of the data concerning fluoride intake and excretion for the Japanese, prior to implementation of water fluoridation in Japan. The measurement of fluoride concentrations in urine as the safety monitoring index of fluoride application for oral health in the community is recommended by WHO. The aim of this study was to investigate the influence of 1mg or 2mg of fluoride burden under the routine life conditions on urinary fluoride excretion. Urinary fluoride excretion until three hours after 1mg fluoride burden or not burden were measured three times over respectively on healthy six adults (average age:35 year-old, five males and one female). Furthermore, urinary fluoride excretion for 24 hours after 1mg or 2mg fluoride burden or not burden were measured once respectively. The results obtained from this study were summarized as follows: 1. The peak of 24-hour urinary fluoride excretion under regular daily life after 1mg or 2mg fluoride burden was observed within three hours after the fluoride burden, and those average quantities of fluoride were 311μg F and 724μg F respectively. 2. The rate of 24-hour urinary fluoride excretion estimated from basal fluoride excretion under regular daily life after 1mg or 2mg fluoride burden were 39.1 and 54.7% respectively. 3. Individual 24-hour urinary fluoride excretion under regular daily life after 1mg fluoride burden varied within the range due to the change of daily foods and beverages intakes. 4. It was suggested that the data of urinary fluoride excretion after fluoride burden estimated from individual basal urinary fluoride excretion would be regarded as the reliable data. As a result of this study, it is concluded that water fluoridation under 1ppm of fluoride adding has no effects on human health and it's implementation in Japan can be recommended as a safe measure for oral health.
