| Abstract ： | 「緒言」現在, 審美部位である上顎前歯部におけるインプラント治療は先端歯科医療におけるトレンドの一角を成している. しかし, 科学的背景に基づいた臨床的アプローチは乏しく, 審美性の長期安定に関する評価・報告も少ない. 今回, 10年経過症例を再評価することにより, 審美領域におけるインプラント治療コンセプトの変遷と治療計画の再評価を行い, 今後同様の症例に対する治療を有効に進める指針とした. 「症例および方法」 1. 初診時年齢 61歳 2. 性別 女性 3. 初診日時 1997年2月12日 4. 主訴 前歯部ブリッジの審美障害. 固定性補綴治療希望(図1) 5. 既往歴 特記事項なし 6. 全身所見 特記事項なし 7. 口腔内所見および現症 両側犬歯を支台にした前歯部ブリッジの挺出による機能・審美障害が見られる. ⊥3の歯周ポケットは7mmで保存不可能であると判断した. 他の部位には大きな問題はない. |

| Abstract ： | Although implant placement in the esthetic zone has become popular, the implant treatment lacks scientific backgrounds and there have been few clinical reports and evaluations. We performed multiple teeth replacement in esthetic zone with implants in 1997, and evaluated 10-years results of the cases in 2007. The results of the evaluation were as follows: 1. In multiple teeth replacement with implants, 1.0-1.5mm horizontal bone resorption from the implant shoulder (so-called“saucerization”) occurs, so the minimum number of implants is recommended. 2. The implant shoulder is rather deep subgingivally to manage soft tissue, so a screw-retained superstructure is recommended. 3. To establish long-lasting labial soft tissue stability, a thick facial bone wall of at least 2mm is essential with sufficient bone augmentation. From these evaluations, we need to set guidelines for multiple teeth replacement with implants. |
