| Abstract ： | 「緒言」 矯正治療で叢生を改善する方法としては, 歯列弓の側方や前方への拡大, 臼歯部の遠心移動, 臼歯の直立, 抜歯などが挙げられる. どの方法を用いて叢生を改善するかを決定するには, 正面および側面頭部X線規格写真による三次元的な情報を元に診断と治療目標の設定を行うことが重要である. 本症例では, 口唇の突出感がなく側方拡大の余地があると診断し, 下顎第三大臼歯の抜去後にシャーミーplateおよびマルチブラケット装置を用い, 歯列弓の側方拡大と上顎右側臼歯部の遠心移動による叢生の矯正治療を行った. その結果, 動的処置終了後4年6ヵ月を経過しても良好な咬合状態を維持しているので報告する. 「症例」初診時年齢15歳4ヵ月の女子で, 叢生を主訴として来院した. 1. 家族歴および既往歴 父親が叢生. 既往歴は特記すべき事項はない. 2. 現病歴 永久歯に生え変わる頃より, 叢生が著しくなってきた. |

| Abstract ： | The patient was a 15-year and 4-month old Japanese female who presented with anterior crowding accompanied by V-shaped form in both the maxillary and mandibular arches. Orthodontic treatment was performed involving extraction of the lower third molars, upper right molar distalization and maxillary lateral expansion using a combined shamy appliance with expansion plate. After 8 months, mandibular lateral expansion was attempted with a bi-helix and 1 month later, the maxillary plate was changed to a quad-helix appliance for better results. Eleven months after the start of treatment, pre-adjusted edgewise appliances were used for arch alignment and occlusion adjustment. Active treatment was completed in 36 months facilitating the establishment of an individual normal occlusion. The patient's profile was almost maintained from the time of the first examination. Dental esthetics and occlusal relationships were satisfactorily achieved through the various stages of treatment, and prevailed through the retention period. |
