Abstract : 「緒言」 多形性腺腫pleomorphic adenoma(以下PAと略す)は唾液腺腫瘍において最も多く認められる良性腫瘍で, その組織像は腺管構造, 粘液腫様組織, 軟骨腫様組織など極めて多彩であることが知られている1~3). しかし脂肪組織の存在は比較的稀であり, 広範囲に多量に存在する場合はlipomatous pleomorphic adenoma(以下LPAと略す)と, 近年定義するようになってきている2). 今回, 我々は口蓋に生じたLPAを経験したので, 文献的な考察を含め報告する. 「症例」 患者:33歳, 女性. 初診:平成17年1月13日. 主訴:左側軟口蓋部の腫瘤. 現病歴:2ヵ月前より左側口蓋の腫瘤を自覚, 当科へ紹介来院した. 既往歴:特記すべき事項なし. 現症 全身所見:体格は中等度. 栄養状態は良好. 口腔外所見:顔貌所見に異常は認められなかった. 口腔内所見:左側軟口蓋部に, 表面平滑, 弾性硬, 1.5×2cm大の腫瘤で, 可動性を認めた.

Abstract : [Abstract] Pleomorphic adenoma (PA) consists of ductal structures and mesenchymal components having myxoid or chondroid appearance. However, lipomatous components are common in PA, despite the wide variation in histological appearance of PA. PA including large lipomatous components is known as lipomatous pleomorphic adenoma (LPA). LPA is a rare bening tumor of the salivary glands, in which the parotid is the least commonly involved site. To the author's knowledge, LPA of the palate has not been reported. This paper reports the case of a 33-year-old female with LPA that presented as a soft palate tumor. The patient visited Kanagawa Dental College Hospital complaining of swelling of the soft palate. A mass measuring 1.5×2.0cm was found on the left side of the soft palate. MRI revealed high intensity on both T1-weighted and T2-weighted images. Histologically, the resected tumor was predominantly composed of lipomatous stromal components. Immunohistochemically, the myoepithelial cells and adipose tissue were positive for S-100 protein. Adipose tissue of LPA is derived from metaplasia of myoepithelial cells.
