Abstract : 「緒言」歯科治療に対して多くの歯科患者は恐怖感や不安感を有している. その軽減を目的として行われているのが精神鎮静法であり, 通常, 吸入鎮静法と静脈内鎮静法に分類される. 精神鎮静法は身体的刺激や呼びかけに対し適切に反応し, 自力で呼吸できる状態を維持したまま患者の精神的緊張を和らげる患者管理法である. 精神鎮静法が適切に行われれば, 非常に効果的な管理法であるが, 歯科では術野が気道と重なるため容易に誤嚥や舌根沈下による呼吸抑制が生じることを念頭におき, 使用する薬剤にも精通しておく必要がある1),2). ここでは歯科治療における精神鎮静法の概念を概説する. 「1. 精神鎮静法の概念」歯科患者の多くは歯科治療に対する恐怖や拒否感を少なからず有し, 特にインプラントなどの外科的な処置に対しては「痛いもの」という先入観とイメージを持っていると思われる. このような恐怖感や不安感は, 精神的な緊張を強くさせ, さらには疼痛性ショックや過換気症候群などの全身的偶発症を発症させる要因ともなり得る.

Abstract : [Abstract] Fear and anxiety are common responses to the dental procedures. Many patients can relax by having dental procedures carried out with conscious sedation administered by the intravenous or inhalation routes. Conscious sedation is defined as a minimally depressed level of consciousness that retains the patient's ability to independently and continuously maintain an airway and respond appropriately to physical stimulation and verbal command and that is produced by a pharmacological or non-pharmacologic method or combination thereof. When appropriately implemented, conscious sedation is a remarkably effective technique. However, dental procedures involve the crucial entry of the airway. Dentists who employ these techniques must be familiar with the pharmacology of the agents used during conscious sedation, recognition of the risks and benefits of the technique employed, and able to manage adverse events that may arise with its use. The clinical method of conscious sedation during dental procedures will be outlined in this paper.
