| Abstract ： | 「緒言」 下顎第三大臼歯は萌出余地がなく骨内に埋伏し, 智歯周囲炎や歯列不正の原因となるなどの理由で抜歯されることが多い. 成人の下顎第三大臼歯抜歯術は口腔外科では日常, 頻繁に行われる手術のひとつであり, その抜歯術に関連する合併症や適応症の報告等は多数ある1～4). しかし, 歯胚段階での下顎第三大臼歯の抜歯については, 矯正歯科から早期抜歯の有用性に関する報告5～7)が散見されるのみで, 合併症, 手術侵襲の程度, 安全性を検討した口腔外科からの報告8～13)は少ない. 今回, われわれは, 歯胚段階での下顎第三大臼歯抜歯の安全性を検討するため, 下顎第三大臼歯歯胚摘出術を行った50症例, 100歯胚について, 歯胚の状況, 手術時期, 麻酔方法, 手術侵襲, 合併症について調査したので報告する. 「対象と方法」 対象は1991年3月から1994年3月までに, 横浜市立港湾病院歯科口腔外科で行われた下顎第三大臼歯歯胚摘出術50症例, 100歯胚である. |

| Abstract ： | The aim of this study was to evaluate surgical invading complications and safety after removal of mandibular third molar germs in terms of oral surgery. This study included a series of 100 lower third molar germectomy procedures performed on 50 patients aged 10 to 16 years. All procedures were performed by one surgeon under local or general anesthesia. General anesthesia was selected for cases of simultaneous multipule teeth extractions. Complications and side effects were observed in three cases; they involved trismus, postoperative infection, and stomatitis. All complications were transient and improved quickly. No severe complications such as mandibular nerve paresthesia and disturbance of eruption of mandibular second molar were observed. We consider lower third molar germectomy could be performed safely in adolescents, similar to how impacted third molar in adults are extracted. |
