Abstract : | 「緒言」現在インプラント治療は歯科治療の中で重要な治療オプションとして日常臨床に取り入れられ普及し, 今後もますます拡大する様相を呈している. このことはインプラント治療の予知性が高いことも普及した大きな要因である. インプラント治療の予知性を評価する目的で, インプラント成功率, 生存率についての報告がなされており, 10年以上の長期研究でも多くの論文でインプラント生存率90%以上の良好な成績を示している1-8). また, 最近では上顎洞骨造成術を伴ったインプラント植立や, 抜歯即時インプラントなどの特殊な状況下での長期にわたる良好なインプラント生存率も報告されるようになった9,10,11). 当医院で使用しているストローマン社製インプラント(Institut Straumann, Waldenburg, Switzerland)の10年以上にわたる種類別生存率は中空型シリンダーインプラント85.7~96.1%, 中空型スクリューインプラント95.4~96.6%と報告され6,12,13), 特に今回分析した充実型スクリューインプラントは99.2%と高い生存率が報告されている2). |

Abstract : | [Abstract] The purpose of this retrospective study was to evaluate 10-year cumulative survival rate of implants placed in completely and partially edentulous jaws. A total of 809 implants placed in 208 consecutive patients between 1995 and 2005 were evaluated with KAPLAN-MEIER method. Solid-screw implants placed were all Straumann implants (Institut Straumann, Waldenburg, Switzerland), with Titanium Plasma Spray surface (n=220) or Sand-blasted, Large-grit and Acid-etched surface (n=589). After implant placement, 1.5 to 6 months of healing was allowed before functional loading by final prostheses. All collected data was tabulated in December 2005. 20 patients (47 implants) dropped out of this study, resulting in a dropout rate of 9.6%. Of a total of 809 implants, 2 implants were lost within a period of 1.5 to 4 months after placement, resulting in early implant loss of 0.24%. A total of 18 implants were lost during the study period due to several reasons. KAPLAN-MEIER method revealed an 10-year cumulative survival rate of 91.3%. Furthermore, the survival rate was calculated by classifying implants into several subgroups according to the implant type, history of periodontal disease and smoking habit. Long-term outcome of Straumann implants was predictable in both completely and partially edentulous patients. The comparative analysis of subgroups revealed that implant survival is affected by smoking habit. |
