Abstract　： 「諸言」開咬には, 不正形態が歯および歯槽骨に限局される歯槽性の開咬, 顎骨の形態異常による骨格性の開咬, またはその混合型など様々な様態がある. 多くの症例では, 口腔周囲筋や舌の機能異常, 指しゃぶりなどの悪習癖が関与している場合が多く, それが難治症例とならしめる要因のひとつとなる1). 開咬による障害には, 顔貌の不調和や咀嚼機能の低下, 発音の異常, 嚥下運動時の異常などがみられるが, これらは生理的障害だけでなく, 精神的影響を及ぼすこともある1). 本症例は, 上顎歯列弓の狭窄を伴った歯槽性の開咬で, 上下顎左右側智歯を抜去後, マリガンアーチワイヤーによる上顎歯列弓の拡大と, マルチループエッジワイズアーチワイヤー(MEAW)と顎間ゴムによる臼歯部の整直と圧下および前歯部の挺出により治療を行った. その結果, 開咬は改善され, 安定した咬合が得られたので報告する. 「症例」「1. 初診時年齢, 性別」35歳4か月の女性

Abstract　： [Abstract] Open bite may occur along with various malocclusions. The patient was a female aged 35 years and 4 months, with the chief complaint of anterior open bite, unstable occlusion and clicks in the left temporomandibular joint. Our diagnosis was Angle Class I on both sides with +4mm over-jet, and -2mm over bite. The molars except the second and third were in the state of alveolar open bite. The upper dental arch was narrow, molars were cross bite, and premature contact was observed on the first premolar and the first molar on the left. The open bite was treated by extraction of the third molar followed by expansion of the upper dental arch using Malligan archwire, upright and intrusion of molars were treated using multiloop edgewise archwire and extrusion of anterior teeth. As a result, her condition was much improved and her occlusion has been stable for two years and eight months since the active treatment was finished.
