Abstract　： 「諸言」閉塞型睡眠時無呼吸症候群(obstructive sleep apnea syndrome, OSAS)1)は, 睡眠中に上気道の閉塞による呼吸停止が頻繁に起こることにより, 深睡眠の減少と睡眠の分断化などの睡眠構築の異常を引き起こす疾患である. また, 頻繁な酸素飽和度の低下を回復するための微小覚醒応答が心臓血管系に負担となり, OSASの患者では, 高血圧, 心臓血管系の障害のリスクが高まることが知られている. このようにOSASは, 生活習慣病の要因として看過できないことや, 日中の傾眠と認知, 記憶などの障害によって, 交通事故や労働災害などを引き起こす原因になり, これらに伴う社会的損失が甚大であることが広く知られるようになってきた2,3). 本邦では2004年4月よりOSAS治療における口腔内装置(oral appliance, OA)の歯科診療保険の適応が認められて以来, 約8年が経過し, OAを長期にわたって使用し続ける患者が増加してきている.

Abstract　： [Abstract] Obstructive sleep apnea syndrome (OSAS) is common and related condition with major social and health implications which can be treated successfully with dental devices that reposition the mandible. Despite its widespread use, the side effects of these devices have not yet been systematically evaluated. The aims of this study were to investigate possible orthodontic side effects following the use of an oral appliance (OA). Two patients diagnosed with OSAS were treated with OA. Cephalometric analyses and dental cast measurements were conducted to identify objectively the skeletal and dental changes caused by OA. Significant retroclination of the maxillary incisors and proclination of the mandibular incisors were accompanied by overbite and overjet, and protrusion of the lower lip. These results suggest that practitioners should pay attention to the possible side effects of OA on the dentition when treating patients with OSAS.
