Abstract　： 「諸言」オルソインプラントを併用した矯正歯科治療が出現して以来1), その強固な固定源はマキシマムアンカレッジの獲得や前歯および大臼歯の圧下等, 従来の矯正歯科治療で困難とされてきた治療の幅を広げたことは周知の事実である. その中でも菅原ら2,3)によって開発されたSkeletal Anchorage System(以下SASと省略)は, フラップオペレーションが必要になるため, 外科的侵襲が大きいリスクを伴うが, 現在多く利用されているマイクロインプラントとは異なり, 歯の移動方向に制限が少ない利点を有すると考えられる. 今回我々は, 下顎右側第一大臼歯および下顎右側側切歯が先天欠如し, 下顎右側第二大臼歯が埋伏した症例に対し, マルチブラケット装置にSASを併用することにより, 有効な治療結果が得られたので報告する. 「症例概要」初診時年齢17歳8ヶ月の女性(図1)で, 挺出した上顎右側第一大臼歯が, 下顎右側大臼歯部歯肉を咬みこむことによる疼痛を主訴に来院した.

Abstract　： [Abstract] The introduction of combined use of implants with orthodontic treatments provides scope for absolute anchorage and, as we have observed, expanded the possibilities for overcoming previous orthodontic challenges with traditional techniques, including the acquisition of maximum anchorage and intrusion of the anterior and posterior teeth. One such implant system is the skeletal anchorage system (SAS) developed by Sugahara. Although the use of SAS requires an invasive surgical procedure for flap operation, the system has the distinct advantage of excellent flexibility in the directions of movement of the teeth over micro-implants, which is now frequently used. In this case study, we report the case of a 17years 8-months-old Japanese adolescent girl with the chief complaint of occlusal pain in the lower right posterior region. She had angle class I malocclusion with crowding. In her right hemimandible, she had congenitally missing first molar and lateral incisor teeth, and also had a distally displaced impacted second molar, which had led to overeruption of the opposing maxillary right first and second molars. This severe, unilateral, localized and abnormal reduction in the vertical dimension was effectively treated with the SAS in combination with a multibracket appliance (.018X.025 Roth type), with a successful outcome.
