| Abstract ： | 「緒言」ヒト鎖骨頭蓋異形成症(Cleidocranial dysplasia：CCD)は, 膜性骨化で形成される鎖骨や頭頂骨ならびに前頭骨等の形成不全や泉門・頭蓋骨の縫合不全を生じる常染色体優性遺伝疾患である. 顎顔面領域では, 乳歯の晩期残存, 永久歯の埋伏・萌出遅延, 過剰歯や歯冠・歯根の形態異常さらにはそれらに伴う咬合機能異常を主症状とする. 今回, 他施設にて長期にわたり咬合の管理と治療がなされ, 最終的な咬合機能回復を依頼された症例に対し, 外科矯正治療により顔貌と咬合の改善を図った一例を経験したのでその概要を報告する. 「症例概要」1. 患者 18歳7か月, 女性. 2. 主訴 前歯が咬みあわない. 3. 家族歴 父親に前歯部開咬を持つ. 4. 既往歴 特記事項なし. 5. 現病歴 某総合病院の産婦人科にて出生後, 鎖骨頭蓋異形成症と診断された. 鎖骨の外側1/2が欠損し, 肩の動きが不良であったことから, 同院整形外科に通院していた. |

| Abstract ： | Cleidocranial dysplasia (CCD), clinically characterized by clavicular hypoplasia or aplasia and delayed closure of anterior fontanelle, is autosomal dominant inheritance disease associated with a gene mutation of RUNX/CBFA1. As a symptom of CCD in maxillofacial region, it frequently appeared dysfunctional occlusion accompanied with various congenitally abnormalities such as persistence deciduous teeth, supernumerary and/or impaction of teeth and morphological aberration of maxilla and mandible. The patients referred to our clinic for reconstructing the optimum occlusion from nearby hospital which performed the management of occlusal problems during a growth phase period. The purpose of this paper is to describe the orthodontic and approach orthognathic surgical treatment of a 18-year-old female patient with a skeletal class IIIopen bite associated with CCD. |
