| Abstract ： | 「諸言」慢性歯周炎は「歯周病の検査・診断・治療計画の指針2008」において歯周病原細菌により生じる, アタッチメントロスおよび歯槽骨吸収を伴う歯周組織の慢性炎症性疾患であり, 発症時期は35歳以降であることが多く, 組織破壊の速度は比較的緩慢であると定義されている. 重症度はプロービングデプス(Probing depth:PD), 歯槽骨吸収度(Bone loss:BL)の程度によって分類される. すなわちPD 3mm以下あるいはBL 30%未満では軽度, PD 4～6mmあるいはBL 30～50%では中等度, PD 7mm以上あるいはBL 51%未満では重度と分類される. 重度慢性歯周炎では歯周組織が高度に破壊されているため, 正常な咬合力が外傷として歯周組織に悪影響を及ぼすことがある. このような二次性咬合性外傷により炎症が修飾され, 歯周炎がさらに重症化すると考えられている. |

| Abstract ： | [Abstract] In this case report, we describe generalized severe chronic periodontitis in a patient, including periodontal, tooth extraction, restorative treatment and maintenance care. A 68-years-old man visited our clinic complaining of desorption of 48 inlay and mobility of 18 and 17 teeth. Patient has 8.0 mm probing depth (PD) in 17 tooth at the first visit. The percentage of tooth sites with PD<4mm was 60.3%, PD<7mm was 7.1%, the rate of bleeding on probing (BOP) was 71.2% and the O'leary's plaque control record (PCR) was 76.0%. After periodontal initial treatment, the percentage of tooth sites with PD<4mm was 6.7%, PD<7mm was none, the rate of BOP was 18.3% and the PCR was 15.6%. Then we performed open flap debridement and autogenous bone graft at 24, 25, 26, 35, 34, 33, 45 tooth, and trisection of bucco distal root at 16 tooth. At oral rehabilitation stage, we provided definitive prosthesis. At maintenance stage, the percentage of tooth sites with PD<4mm was none, the rate of BOP was 6.7% and the PCR was 7.5%. We performed maintenance every month, which appears to be sufficient for effective periodontal tissue maintenance. |
