Abstract : 「緒言」歯科矯正科に来院する不正咬合治療を希望する患者の中には, 自覚症状が認められない関節円板転位や変形性関節症(Osteoarthritis:OA)を伴うものが散見される. 本症例は, 上顎前歯の前突感と顎顔面・頭頸部筋肉の違和感を主訴に来院した. 検査の結果, 前歯部の早期接触と下顎頭の後方偏位を伴うOAが認められた症例で, 矯正治療による適正下顎位の獲得により, 筋症状の緩和と下顎頭内部構造の変化が画像解析より認められたためここに報告する. 「症例概要」1. 初診時年齢・性別 43歳7か月の女性. 2. 主訴 上顎前歯の前突感と顎顔面・頭頸部筋肉の違和感を主訴に来院した. 3. 既往歴 以前, 顎が痛んだことや両側顎関節部にひっかかりなどを感じたことがあったが, 自然に緩解したため, 受診には至らなかった. しかしながら, 加齢と共に顎顔面, 頭頸部の自発痛の筋症状が強くなり, 現在では朝夕に症状が発現することが増加した.

Title : Image evaluation of morphological change in the mandibular condyle accompanying elimination of functional mandibular displacement

Abstract : [Abstract] The patient had functional mandibular displacement accompanied with condylar deformity which was thought to be caused by initial contact in the anterior tooth area for a long time, and was treated by an orthodontic approach for establishing individual occlusion. In this case, imaging analysis showed that the internal structure of the condyle was continually remodeling due to the change of occlusion after retention for one year. The patient was a 43 year 7 month old female with the chief complaint of upper protrusion and facial and neck muscle stiffness. Functional analysis of the articulator indicated that the right condyle was compressed anterosuperiorly by 1.5 mm and the left condyle was compressed postero-superiorly by 1.5 mm from the reference condylar position (RCP) to ICP respectively by the initial contact of anterior teeth. Both temporomandibular joints (TMJ) indicated disc displacement without reduction and deformity of condyles; in particular, the internal trabecular structure of the left condyle was decreased and the structure of the right condyle with osteophyte formation was observed by MRI and CT findings. The patient was diagnosed as crowding accompanied with osteoarthritis (type IV) of bilateral TMJ. A splint was set and improvement of muscle symptoms was observed. Therefore, the patient underwent orthodontic treatment for 1 year 9 months and retention for 1 year. After treatment, the muscle symptoms disappeared and the internal structure of the mandibular condyle was continually remodeling. These findings indicated that the mandibular condyles adapted to the new mandibular position.
