| Abstract ： | 「諸言」移転歯とは歯の形成期あるいは萌出期に何らかの原因により, 歯がその本来の位置から著しく異なった位置, あるいは互いに交換した位置に萌出している状態を言う. 歯の移転の原因は, 乳歯の早期喪失および晩期残存, 歯胚の位置の異常あるいは発育時の萌出方向の偏位などが考えられている. さらに, 歯胚発育初期の障害や遺伝的要因によるものであるとする報告もある. この移転歯の発現頻度について名和らは0.28%, Ciarlantiniらは0.38%といずれも極めて低い値と報告しており, その発現部位としては上顎犬歯のみであったとの報告がなされている. 日常の臨床において, このような移転を伴う症例に遭遇した場合, これらの位置の交換したまま通常の配列順序とは異なる配列を行うか, あるいは本来の位置に戻して配列するかを選択する必要がある. 本症例では, 上顎両側犬歯と第一小臼歯の移転および上顎両側側切歯の先天性欠如を伴う症例に対し, 歯を本来の位置に戻さずに配列して治療を行ったところ良好な結果が得られたので報告する. |

| Abstract ： | [Abstract] Transposition is a dental anomaly characterized by the exchange of position between two adjacent teeth. Transposition of the maxillary canine and first premolar has a low prevalence in the population. The aetiology of the transposition remains unclear, although it has been associated with genetic factors. In these treatments, uncorrected, the results are often both functionally and esthetically unsatisfactory.Acceptance of the malpositions, considering the treatment term and root resorption. In either case, the cost-benefit must be considered as it may influence the treatment strategy and the decision on whether to accept or correct the situation. This case report presents an unusual approach that was used to treat a 16-year-old patient with a bilateral caninepremolar transposition and a missing maxillary lateral incisor. We chose unusual position treatment, acceptance of the malpositions and positioning of the premolar instead of the lateral incisor. Toward the end of the treatment, esthetically satisfactory and functional occlusion was achieved. The 2-year post-treatment follow-up records show the stability of this treatment. |
