Abstract : 「1. 唾液検査から新しい医療の創生」患者負担を軽減し, 治療効果を最大限に高めることが医療の進歩である. しかし, 効果の方に注目される傾向があり, 患者負担を軽減するための方策は余り見えない. すなわち患者に優しい医療はまだまだ不十分といえる. 唾液検査の実用化は, 上記の問題解決に貢献できる. 現在, 検査の基本は採血であり, この採血からの解放は国民が真に望む医療の進歩であることは疑いなく, 唾液が血液に代わることができれば, 検査医療にイノベーションをもたらすことができる. さらに, 体温計, 体脂肪計, 体重計, 血圧計が家庭に普及したように, 在宅医療やヘルスケアモニタリングに唾液を用いた検査を導入することで, 予防医療に大きく貢献できるはずである. 「2. 唾液検査研究の現状」国内において唾液を検査に応用する研究が, 世間から大きく注目されるようになったのは, 1990年代後半に唾液を用いたストレス測定に関するものである.

Abstract : [Abstract] Prostate Specific Antigen (PSA) is well-known as a tumor marker of prostate cancer. PSA test is useful for screening of prostate cancer or follow up after therapy. Moreover, saliva includes many components that are derived from blood because acinar cells produce saliva using blood materials (plasma). For the above reasons, there has been an increasing focus on the search for saliva biomarkers that might reflect the status of systemic health or disease. Since the collection of saliva is a non-invasive procedure identification of biomarkers in saliva could lead to the establishment of a non-painful procedure. Therefore, there is currently much interest in developing such laboratory tests in medicinal fields. We examined whether there is an association between saliva PSA and serum PSA in patients with prostate cancer. There were significant differences in saliva PSA concentration between a low-serum PSA concentration group (n=20)(<2.5ng/mL) and a high-serum PSA concentration group with high risk of recurrence or metastasis (n=11)(<=2.5ng/mL)(p<0.05). Saliva PSA concentration correlated very well with serum PSA concentration in the high-serum PSA concentration group (γ=0.910, p<0.001) using Spearman's rank test. These results suggest that saliva PSA test is a useful detection of prostate cancer.
