Abstract　： 「緒言」神奈川歯科大学附属病院矯正科は三浦半島の中央に位置する横須賀市において1964年の開設以来, 現在までの約50年間, 歯学部附属病院として神奈川県南部の歯科医療に対する一翼を担ってきた. その間, 国民の生活環境が向上し, 矯正歯科治療に対する社会的認識も大きく変化してきた. 口唇裂・口蓋裂患者に対する治療も1982年には口唇裂・口蓋裂に起因する不正咬合に対し保険適用がなされ, 矯正歯科治療に対する患者負担が軽減された. また, 術前顎矯正治療や口蓋裂閉鎖手術方法の改善, 顎裂部への骨移植, チームアプローチによる早期からの口腔・咬合管理など, 口唇裂・口蓋裂患者への歯科治療は大きく向上した. さらには口唇裂・口蓋裂に加え, 2002年にはゴールデンハー症候群等の6疾患に起因する咬合異常に対する矯正歯科治療が保険適用となり, 2015年の現在では46種類の先天性疾患に拡大されたこともあり, 各地域の様々な施設において矯正歯科治療が行われるようになった.

Abstract　： [Abstract] A clinical analysis was carried out at Kanagawa Dental University Hospital of patients with cleft lip and palate who received orthodontic treatment during the 15 years between January 1998 and December 2012. The findings were as follows: 1. The total number of patients with cleft lip and palate was 34, accounting for 0.9% of all orthodontic patients. The ratio of male to female patients was 1:1. 2. Most patients were residents of Yokosuka City, where our hospital is located. 3. Cleft morphology was as follows: unilateral cleft lip and palate was observed in 35.3%, and bilateral cleft lip and palate and cleft palate in 14.7%. 4. The mode age at initial diagnosis was 8 years (14.7%), followed by 7 years, 10 years and over 20 years (11.8%). 5. The average values of SNA, SNB and ANB angles were 77.6°, 74.2° and 3.5°, respectively. 6. Patients with crossbite accounted for 50.0%. 7. The preferred orthodontic appliance was the Porter arch type slow expansion appliance. There is an urgent need to maintain the system of accepting patients and disseminating public information to inhabitants and medical institutions, because there are very few patients with cleft lip and palate at our hospital. We will strengthen cooperation with each medical institution as an area base hospital, and will contribute to community medicine in the future by serving as a hospital affiliated with a dental university in the Miura Peninsula district.
