Abstract : 「1. "災害医療歯科学"への取り組み」本原稿執筆中には, 阪神淡路大震災から20年, 新潟中越震災から10年半, また東日本大震災から4年が経過しようとしている. 被災された皆さま方には心よりお見舞い申し上げるとともに, 被災地域の早い復興を願い, そのための支援をさらに継続していきたいと考えている. 我が国での地震発生は多く, 近年(1994~2003年)での世界のマグニチュード6以上の地震回数の22.9%を占めることから, 内閣府では災害応急対策についての検討を常に重ね, 国民に減災を呼びかけている. また, 文部科学省研究開発局地震・防災研究課の地震調査研究推進本部は, 報告書「全国地震動予測地図2014年版~全国の地震動ハザードを概観して~」を公表し, 2014年版の確率論的地震動予測地図を示した. とくに, 当センター近隣の湘南・三浦半島地域では, 伊勢原・衣笠北武・武山断層で強い地震が近い将来に予想されている.

Abstract : [Abstract] Immediately after a disaster (in the first 72 hours; Phases 0-1), we must analyze the nature of the situation, determine what is needed and how to proceed, establish goals and action plans, and wait for medical support from outside the affected area. Later (after 72 hours, phase 2 and beyond), adjustments will be made to the medical care and health activities offered based on the actual situation in the affected region. And immediately after a disaster strikes, it is essential to collect and analyze information, to ascertain the demand for medical care and health care activities, and to establish systems for receiving personnel and material support from outside of the affected area. The Japan Dental Association (JDA) trains "Co-ordinator Dentists for a Disaster" who are well versed in dentistry to play a role in tasks ranging from providing dental treatment to disaster victims immediately after a disaster, to coordinating oral health measures aimed at preventing "disaster-related illnesses" and "disaster-related deaths" that can occur soon after a disaster. Disaster-related illnesses include high blood pressure, diabetes, and inactive lifestyle diseases (including some non-communicable diseases: NCDs), and the causes of disaster-related deaths include malignant neoplasm growths, cerebrovascular disease, and particularly among elderly people in many cases, pneumonia. People tend to have poor oral health after a disaster due to subsequent water shortages, dehydration, and stress, and oral bacteria increase as people continue wearing unclean dentures. Occurrences of aspiration pneumonia caused by immunodeficiencies due to poor nutrition and a lack of exercise are also prevalent. Thus, active oral health care practices during a disaster can be linked to "protecting the mouth as well as the entire body". Japanese dental professionals play a role in conducting community outreach not only during emergencies, but also during non-disaster times on a regular basis, providing ongoing support for the dental treatment and oral health of all Japanese people.
