Abstract　： 「緒言」2011年3月11日に発生した東日本大震災に対して, 県や市町村レベルで避難所や被災地域へ全国各地から様々な支援が展開された. 医療支援については, 阪神・淡路大震災や地下鉄サリン事件などを契機に整備された災害拠点病院や災害派遣医療チーム(DMAT)によって緊急医療支援が展開されたが, その一方で公衆衛生や保健に対しては, 医療支援チームと比べ, その人数は少なく対応能力にも大きな差があったと指摘されている. 歯科からは, 齲蝕や歯周病に対する応急処置や義歯への対応などの「歯科治療」, 歯型照合による「身元確認作業」とともに, 口腔ケアを中心とした「歯科口腔保健医療」が提供された. これらの中で, 口腔ケアによる震災関連疾患としての肺炎予防は, 高齢者の命を守るケアの1つとして非常に注目されている. 肺炎は本邦における死因の第3位で, その多くは高齢者である. 高齢者の肺炎は, 就寝中の不顕性誤嚥が原因の場合が多く, デンタルプラークを含む唾液が大きな要因となる.

Abstract　： [Abstract] Against large-scale disaster, prevention of aspiration pneumonia as disaster-related death by oral care, has received much attention as one of the care to protect the lives of elderly people. We must support requiring long-term care elderly people during large-scale disasters. We have conducted the survey for arrangement of long-term care insurance facilities in oral health care management system during large-scale disasters at Yokosuka-shonan area, Kanagawa-prefecture. Yokosuka-shonan area have small flatland, steep slope area, close proximity to houses, many tunnels and long coastline. We are worried about the possibility of landslides by active fault and liquefaction of the ground. So, we must build a medical cooperation of oral health in long-term care insurance facilities during large-scale disasters.
