Abstract : 「緒言」 一過性全健忘(transient global amnesia; TGA)は急性発症の強い前向性健忘と逆行性健忘を主徴とする. 見当識や自己認識は比較的保たれるが, 発作中は同じ質問を繰り返す. 記憶障害は一過性で多くは24時間以内に消失し, 随伴する神経学的徴候は認めないことが特徴である. TGAの発症機序は不明な点が多く, 約50%の症例で肉体的, 精神的ストレスなどの誘因を有すると報告されている. TGAは各種臨床検査のみで診断することはできず, 詳細な病歴聴取, 身体所見評価, 認知機能評価が必要であるとともに健忘の原因となり得る疾患の除外診断が重要であり, TGAの診断にはHodgesらの診断基準が用いられる. 今回, われわれは神奈川歯科大学附属病院での歯科治療直前にTGAを発症した症例を経験したので報告する. 「症例」 症例: 73歳女性. 主訴: 健忘. 現病歴: 201X年8月, 炎天下に自宅の庭仕事をした.

Abstract : [Abstract] Transient global amnesia (TGA), a clinical syndrome, is characterized by transient loss of recent retrograde and anterograde memory, with preservation of immediate recall, remote memory, visual-spatial function, attention, and language. Most patients are >50 years of age and rarely exhibit other neurological deficits. Symptoms resolve within 24 hours, recurrence is rare. The etiology is not fully understood. We describe a case of TGA that occurred before dental treatment. A 73-year-old woman was referred to the Department of Internal Medicine at Kanagawa Dental University Hospital because of acute forgetfulness. One day before visiting the department, she did gardening during the hot daytime of August. The next day, she came to our hospital for dental treatment. At that time, the dentist became aware of her acute forgetfulness. Neurological investigation did not reveal any pathological findings. However, repetitive questioning was observed. She repeatedly asked, "Where am I?" Blood examination revealed elevated level of low-density lipoprotein cholesterol. However, complete blood cell counts, level of plasma glucose and electrolytes, thyroid function and adrenal function were all within normal limits. Computed tomography (CT) scan of the brain was normal. Here memory improved and reverted to normal after 24 hours at home. Electroencephalography and CT of the brain were performed in another hospital the next day, and the findings were normal. Finally, she was diagnosed with TGA. It was suggested that the cause of her TGA might be physical stress from working outside in the heat. The incidence of TGA is reported to be 3.4-10.4 per 100,000 people. In population >50 years of age, the incidence increases to 23.5 per 100,000 people. Thus, all clinicians, including neurologists, physicians psychiatrists, and dentists, need to be aware of patients >50 years of age who may be at higher risk for TGA.
