Abstract : 「緒言」 超高齢社会を迎えた日本では, 世界的に見ても高齢化のスピードは早く, 人口の重心が高齢化へと移動している. それに伴い疾病構造に変化が生じており, 日本の医療のあり方についても変化が求められている. そのような中で, 個人のQOLをいかに低下させずに健康寿命を延ばすかが課題となっている. しかしながら, 生活習慣病などの全身疾患の有病者数は年々増加し, 近年ではその全身疾患と歯周病などの口腔疾患との関連性が社会的に問題視されてきている. また, 最近の研究では認知症やフレイルも生活習慣病と考えられてきている. 特に高血圧や糖尿病, 脂質異常症などは全身における循環障害を誘発するため, 歯周組織においてもその影響は大きく, インプラントなどの歯科治療においてはそれら疾患が弊害となる. われわれはこれまでに歯周組織の微小循環に注目して研究を行ってきた. 歯周組織においては多数の血管網が存在しており, ヘアピン状の毛細血管ループが規則的に認められる.

Title : Effects of circulatory disturbance associated with aging on the brain and oral cavity

Abstract : [Abstract] In Japan, which has entered a super-aging society, changes in the disease structure and an increase in the number of people requiring long-term care are problems. In recent years, the relationship between systemic diseases and oral diseases such as periodontal disease has become a social problem. In this study, we focused on the microcirculation, which play a key role in the onset and pathophysiology of various lifestyle-related diseases, and investigated the circulatory disorders of the brain and oral cavity caused by aging. Aged male rats (F344/Jcl, 22-months-old) were used in this study. To perform morphological analysis of blood vessels, resin cast model was prepared, and blood vessels around the brain and periodontal tissue were observed with a scanning electron microscope. Motor function was analyzed using an open-field test. Functional analysis in angiogenesis was performed by measuring blood and salivary concentrations of VEGF by ELISA. In the experimental group, the vascular loop in the gingival tissue was poor, and the blood vessels showed distorted morphology. Furthermore, in the open-field test, the number of entries into the center-area was significantly lower, and the total distance traveled tended to be shorter. The saliva and blood VEGF tended to be high in the experimental group. Consequently, this study found that the motor function was decreased in the aged rat, and that the microcirculation in the periodontal tissue and the brain was poor. Therefore, it was possible to confirm the disorder of the systemic function and the microcirculation associated with aging. In the future, we will improve the decline in motor function and impaired microcirculation, examine the relationship between the whole body and the oral cavity, and analyze what kind of effect it will have on the improvement and prevention of diseases.
