Abstract : 「緒言」 口唇口蓋裂の治療は, 生後間もなく形成外科や歯科口腔外科での手術をともなう治療からはじまり, 小児科, 小児歯科, 矯正歯科, 耳鼻咽喉科, 言語治療などとの長期間にわたる連携を要する. そのため患者ならびに保護者は治療に対する意識を長く持ち続けることが必要となる. しかし現実には就学期, 思春期, 受験, 就職等の個人や家庭の抱える複雑な問題もあり, 治療の中断や治療計画の修正などを要することも少なくない. このことに関連して高尾らは保護者が患者へ疾患を伝える状況, 熊谷らは家族内の心理, 北尾らは学童期の教育現場で生じる問題などについて治療が中断するいくつかの関連要因を提示している. 今回, 著者らは混合歯列期(8歳7か月)から治療を開始したが, 思春期性成長発育期での治療の中断(11歳9か月)を経験し, その後4年3か月間の中断を経て(16歳0か月)治療を再開することになった症例を得た.

Title : Long-term management of bilateral cleft lip and palate : A case of interrupted orthodontic treatment during adolescence

Abstract : [Abstract] The treatment of cleft lip and palate usually begins with plastic or oral surgery, followed by long-term management into adulthood that requires collaboration with specialists in pediatrics, pediatric dentistry, orthodontics, otolaryngology, and speech therapy. However, these collaborations don't always go smoothly, and treatment is often interrupted. Herein, we report a case of a 8-year-and-7-months-old male patient whose treatment started in the mixed dentition stage but was interrupted during adolescence (at 11 years and 9 months of age). Treatment was resumed at the age of 16 years and 0 months. The patient had an ANB (relative position of the maxilla to the mandible) of +6.0 degrees during the mixed dentition stage, but after resumption of treatment, the ANB was -3.0 degrees, requiring a major modification in the treatment plan. As a result, Le Fort I osteotomy and sagittal split ramus osteotomy were prescribed. The patient was placed on retainers after 6 years and 9 months (at 22 years and 9 months of age) of treatment. The occlusion was confirmed to be stable after 2 years and 6 months of follow-up (at 25 years and 3 months of age), and the patient's hospital visit was completed after 16 years and 8 months of long-term management. In this case, we report on the factors that led to the discontinuation of treatment, the changes in the treatment plan that occurred as a result of the discontinuation, and the treatment process and results.
